AN ALTERNATIVE APPROACH PRESERVING THE LARYNGEAL FRAMEWORK FOR EARLY STAGE GLOTTIC CANCER SURGERY: LARYNGEAL EVISCERATION ( SUBPERICHONDRIAL INTERNAL VISCERATION)

KBB Kliniği, Ankara, Türkiye We present an alternative surgical approach (subperichondrial internal evisceration) preserving the laryngeal framework for early glottic cancer in this paper. 19 patients (T1a, n=6; T1b, n=4; T2, n=9) were operated on by using the subperichondrial internal evisceration approach. Data collection included decannulation time, feeding tube removal time, local control, and voice result. The mean decannulation time was found to be 6 days, while the mean feeding tube removal time was 8 days. No local recurrence was observed in any of the patients. The mean follow-up period was more than 1 year for 12 patients and lower than 1 year for 7 patients. Subjective voice quality was satisfactorily in all patients. Absence of any local recurrence and satisfactory functional results has suggested that subperichondrial evisceration approach is an effective treatment method. Among the advantages of this technique are; 1) accession to the anterior commissure, subglottic region and paraglottic region under direct visual control, 2) the operation’s being a single phase operation despite its allowing for a very wide resection in horizontal/vertical plane and its unnecessitating any additional reconstructive applications.
